Discharge Medicines Service
When you're ready to leave the hospital, you can take part in our Discharge Medicines Service. This service puts you in direct contact with our pharmacist, allowing us to support you with your medicines after discharge.
Your discharge information is sent to us electronically by the hospital, enabling us to review your medication and prepare everything in advance.
We will check your medicines, ensure any changes are correct, and provide guidance to help you take them safely.

Our pharmacist will review all of your medicines with you after you leave hospital, making sure everything is safe, correct, and easy to understand.
We will:
Explain any new medicines and changes made during your hospital stay
Check that your medicines are safe and suitable
Provide clear advice on how and when to take them
Answer any questions you may have
Taking medicines incorrectly can affect your health. Our aim is to help you understand your treatment and feel confident managing your medicines at home.
This is a free NHS service, and we work closely with your GP to ensure continuity of care.
